There may be times when we would like to include you or your dependent on our website or social media pages through postings and/or print or visual media. For instance, when braces are removed, we may want to post a photo of the patient’s winning smile on our Facebook page! Please review and submit our online “Social Media Consent Form.” Do not hesitate to contact us if you have questions.
